Student/Family Application

Student enrollment for 2026 is now open! Parents interested in enrolling their student(s) in the Academic Support, Enrichment and Mentoring Program must complete the student/family  application form found below. Once it has been submitted the application process begins and will end with setting an actual enrollment date.  Program staff will contact the Family Leader to establish appointment times for completing assessments and forms.  The start date for participating in the program is set once all the steps have been completed and per the availability of a tutor match. Applications will be held in waitlist status while waiting for a tutor match.  Waitlist families are contacted at the start of  each new term for continued interest in enrollment. For more information about the  program and services provided visit the What We Do page.​
