Student/Family Application

Student enrollment for 2025 is now open! Parents interested in enrolling their student(s) in the Academic Support, Enrichment and Mentoring Program must complete the student/family  application form found below. Once it has been submitted the application process begins and will lead to setting an enrollment date.  Program staff will contact the Family Leader to establish appointment times for entering into the enrollment process.  The start date for participating in the program is set once all the steps have been completed and the availability for a tutor match. For more information about the  program and services provided visit the What We Do page.​
