High School Application

Even though the Student/Family Application must be completed, we are asking High School students to complete this additional form. Sometimes a student wants to enroll themselvs and so we'll contact the Family to ask for the family form to be completed. Sometimes the Family Leader enrolls a student but the they aren't really interested in the additional services for the high school group.  This form helps to ensure we're all in agreement in giving the high school group exactly what they need. Please have your student complete the information below and we'll notify them of the meetings for EOP and assure they are matched with a MathUp! level tutor, a content rich person who can tutor Algebra I and II, Geometry, Calculus, and Trignometry. ​
